Job summary

CenterWell Senior Primary Care is seeking a Primary Care Physician to deliver comprehensive primary care to seniors in an outpatient setting. You will collaborate with an interdisciplinary team and document care in the EMR Athena, dedicating 100% of your time to direct patient care with structured administrative components.

The role is on-site in an outpatient office, reporting to the Associate Medical Director, with a focus on high-quality, personalized care for seniors and a supportive

Qualifications

• Graduate of accredited MD or DO program from an accredited university.
• Board Certification or Eligible (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine.
• Current and unrestricted medical license or willing to obtain a medical license in state of practice.
• Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
• Active and unrestricted DEA license (preferred).
• Medicare and Medicaid Provider Number (preferred).
• 2+ years of experience (preferred).
• 2+ years of experience with Medicare Advantage Patients (preferred).
• Knowledge of Medicare standards (preferred).
• Knowledge of HEDIS quality indicators (preferred).

Responsibilities

• Provide comprehensive Primary Care to seniors
• Collaborate daily with the interdisciplinary team.
• Document in EMR Athena
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time.
• Work on-site, in an outpatient office setting
