Job summary

Humana CenterWell Senior Primary Care is seeking a Nurse Practitioner or Physician Assistant to join a collaborative care team serving seniors. You’ll deliver comprehensive, value-based care as part of an integrated team of physicians, pharmacists, and care coordinators.

This role emphasizes ample patient interaction, ongoing professional development, and a supportive culture with generous PTO and CME time. 40 hours weekly.

Qualifications

• Master’s Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Strong communication and interpersonal skills.
• Passion for value-based care and integrated team-based models.
• Basic computer proficiency (EMR, email).
• Preferred: experience in value-based care or Medicare Advantage settings.
• Preferred: DEA license and Medicare/Medicaid numbers or ability to obtain them.

Responsibilities

• Provide comprehensive care to seniors in accordance with clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Participate in daily huddles and on-call rotation.
• Document care accurately with support from quality-based coders.
• Support growth initiatives at your center and ensure licensing compliance.
