Job summary

Conviva Senior Primary Care in Jourdanton, TX seeks a Nurse Practitioner or Physician Assistant to join a collaborative, multidisciplinary team focused on value-based care for seniors. You will work onsite, reporting to the Associate Medical Director, and participate in on-call rotation.

The role requires an active NP or PA license in Texas, a Master's Degree in Nursing or PA program with board certification, and a TB screening. Eligible for a bonus incentive plan depending on performance.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active NP or PA license in the state of practice.
• TB screening required for selected candidates.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Demonstrate passion for value-based care.
