Job summary

Responsibilities

• Conduct thorough evaluations of residents' physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Utilize therapeutic techniques and equipment to achieve treatment goals.
• Monitor and document residents' progress and adjust plans as necessary.
• Work with physicians, nurses, and occupational therapists as part of the care team.
• Communicate treatment plans and progress to residents and families.
• Participate in interdisciplinary team meetings and care planning sessions.
• Maintain accurate documentation of patient interactions and treatments.
• Ensure compliance with federal, state, and local regulations and facility policies.
• Participate in quality assurance and performance improvement activities.
• Provide education and training to residents and families on exercises for home use.
• Offer support to residents to help achieve rehabilitation goals.
