Qualifications

• Must be a clinically-active, board-certified physician at ASV Kokomo hospital.
• Licensure/ Certification/ Registration: Indiana Medical Licensing Board or Indiana Board of Podiatric Medicine credentials before hire date.
• Education: MD/DO or equivalent as listed above.
