Legacy Healthcare Services in Texas is seeking a Physical Therapist (PRN) to provide rehabilitative services to senior residents as part of a coordinated care team. The role involves planning treatment programs, documenting progress, and delivering value through private sessions focused on functional goals.

The position requires current PT licensure in the state, strong interpersonal and clinical skills, and a minimum commitment of 8 hours per month.

Qualifications

• Excellent interpersonal, fiscal, and clinical skills.
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment.
• Degree in area of discipline.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, for individualized attention through a functional-based treatment approach.
• Ability to individualize treatment sessions.
