Legacy Healthcare Services is seeking a Physical Therapist (PRN) to join a compassionate team serving senior living residents. The role emphasizes individualized, one-on-one care and building patient relationships within a coordinated care plan.

Qualifications

• Degree in Physical Therapy; valid state license or ability to obtain before start.
• Strong interpersonal, clinical, and computer skills.

Responsibilities

• Provide rehabilitative services to patients under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one on one with individualized private treatment sessions and functional-based approaches.
• Customize treatment sessions to meet each patient’s needs.
