Job summary

Legacy Healthcare Services is seeking a Physical Therapist for a PRN position within senior living communities, focused on one-on-one, functional-based therapy to improve residents' quality of life.

You will plan individualized treatment programs, participate in clinical conferences, and enjoy a Monday–Friday schedule with no weekend or holiday requirements, plus comprehensive benefits and licensure reimbursement.

Qualifications

• Degree in Physical Therapy and eligibility for licensure in the state of practice
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency and familiarity with EHR systems

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for residents.
• Treat residents one on one with private treatment sessions and functional-based approaches.
• Individually tailor treatment sessions to patient needs.
• Support a Monday–Friday schedule with no weekend or holiday requirements
