Qualifications

• Licensed Physical Therapist in California.
• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Proficiency with medical records and policy interpretation.
• Experience with Net Health and PointClickCare systems.

Responsabilités

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT assistants and staff in resident care.
• Communicate progress with physicians, families, and team members.
• Complete timely documentation and ensure compliance with regulations.
• Support infection control and equipment maintenance.
• Educate residents and families on home programs and fall prevention.
