Luxor at Sayville Rehabilitation and Nursing Center is seeking an experienced Physical Therapist to join our facility team. You will assess referred patients, develop and administer treatment plans, and document progress to support optimal rehabilitation outcomes.

You must be a graduate of an accredited Physical Therapy program, hold a current license, and be comfortable supervising Physical Therapy Assistants.

Qualificações

• Graduation from an accredited Physical Therapy program.
• Current professional license/registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertification and discharge documentation.
• Collaborates with disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.

Responsabilidades

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment plans with goals per patient capacity.
• Document patient condition and progress; educate patients and families.
• Supervise and oversee Physical Therapy Assistants.
• Adhere to confidentiality, HIPAA, and residents' rights.
