Central California Alliance for Health is seeking a temporary Complex Case Management Specialist (RN) to report to the Complex Case Management Supervisor - Pediatrics. The role focuses on developing individualized intervention plans for members in the Case Management program and coordinating with interdisciplinary teams to improve outcomes and control costs.

The position is a full-time, 40-hour-per-week temporary assignment (Nov 2, 2026–Dec 31, 2026) with a hybrid work arrangement.

Competenze

• Master's Degree in Social Work
• Three years of Social Service experience in an acute care setting, community agency or managed care environment
• Knowledge of Medi-Cal policies and regulations

Mansioni

• Develops and manages an individualized intervention plan for members referred into the Case Management program with the goal of promoting optimal, achievable outcomes in the most cost effective and appropriate manner
• Works with the interdisciplinary Case Management team to educate members, providers, external agencies, and internal departments and assisting in navigating the managed care system
• Participates in Quality Improvement efforts to evaluate the Case Management Program's effectiveness and outcomes
