Job summary

Oasis Therapy Services in Madison, WI, is seeking a part-time Occupational Therapist to provide one-on-one rehabilitation across assisted living facilities in Madison and Beaver Dam. The role focuses on ADLs, mobility, safety, and independence, with progress monitoring and caregiver education.

Qualifications

• Active and current Wisconsin Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Reliable transportation and ability to travel between both facilities.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.
