Job summary

Oasis Therapy Services in Beaver Dam, Wisconsin, seeks an Occupational Therapist to provide one-on-one rehabilitation in an assisted living/memory care setting. The position offers part-time hours with a focus on individualized patient care and close collaboration with the care team.

The role requires an active Wisconsin OT license, completion of an accredited OT program, and CPR certification. Experience in geriatrics or similar settings is preferred, with opportunities for professional growth

Qualifications

• Active Wisconsin OT license required
• Graduate of an accredited Occupational Therapy program
• Current CPR certification required
• Strong communication, organization, and documentation skills
• Geriatric, assisted living, skilled nursing, or rehabilitation experience preferred

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide therapy focused on ADLs, mobility, safety, and independence
• Monitor progress and adjust treatment plans as needed
• Educate residents and caregivers on exercises, adaptive techniques, and equipment
• Complete accurate Net Health documentation and collaborate with the care team
