Job summary

Bon Secours Hematology & Oncology in Greenville, SC is seeking an Advanced Social Worker to provide high-level psychosocial support, counseling, and advocacy for patients and families facing complex medical challenges.

The role collaborates with a multidisciplinary care team, conducts needs assessments, develops individualized care plans, and coordinates resources. Requires MSW and state licensure; 2 years of clinical experience preferred.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Licensed Social Worker (LSW) in state of practice required.
• 2 years in social/human services with direct practice and treatment planning.

Responsibilities

• Evaluates patients’ needs and develops individualized care plans.
• Provides counseling and education to help patients cope with changes.
• Manages caseloads and refers to community resources as needed.
• Advocates for patients and coordinates with multi-disciplinary teams.
• Responds to crises and connects patients to appropriate services.
• Collaborates with providers, therapists, and clinical staff to coordinate care.
