Responsibilities

• Screen applicants to determine eligibility by assessing income, residency and nutritional risk.
• Prescribe food packages and provide group and one-on-one nutrition education on healthy eating and breastfeeding promotion.
• Identify need for individual care plans and implement them for high-risk participants; refer to other health and social services and provide follow-up.
• Document nutrition services provided and maintain participant records; travel to clinics and participate in outreach events to increase caseload.
