Job summary

Optum Home & Community Care seeks a Senior Advanced Practice Clinician to join the SCC team, delivering comprehensive assessments and care coordination for high-risk health plan members in long-term care settings. You'll partner with interdisciplinary teams to ensure seamless, patient-centered care and effective care planning.

Required NP/PA certifications, active state licensure, DEA (as needed), and reliable transportation.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state
• Active and unrestricted driver's license with reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain
• Proven ability to gain a collaborative practice agreement
• Proven ability to navigate varied environments and perform duties

Responsibilities

• Conduct comprehensive assessments including medical history review, vital signs and exams
• Support members during short-term sub-acute rehabilitation
• Ensure accurate documentation including ICD-10 conditions
• Collaborate with interdisciplinary care team
• Lead advanced illness and end-of-life planning conversations
• Provide counseling and education to patients and caregivers
