Job summary

Horizon Services, Inc. is seeking a Lead Nurse Coordinator in San Jose, CA to screen participants for suitability to enter the Sobering Center and to facilitate medical and mental-health triage. You will coordinate definitive care and support clients as they transition toward recovery.

The role requires hands-on health screenings, monitoring vitals, documenting observations, and crisis intervention. A sign-on bonus of $500 is offered for the right candidate.

Qualifications

• Screen participants for suitability to enter the Sobering Center.
• Facilitate medical and mental-health triage.
• Coordinate definitive care and transition to recovery.

Responsibilities

• Perform health screening and rapid risk assessment for all clients.
• Monitor vitals and medical triage.
• Maintain records of client medication in logbook or EHR.
• Document vitals, observations every 30 min, and mandatory case notes every 6 hrs.
• Perform conflict resolution and crisis intervention.
• Respond to emergencies and urgent client needs.
