Job summary

Trinity Health is seeking a Nurse Senior – Complex Care Specialist to coordinate highly complex inpatient care within the Acute Care Management team. The role emphasizes leadership, care planning, and seamless transitions for acutely hospitalized patients with multifaceted needs.

You will work with care teams to remove barriers to progression and support timely discharge planning, while upholding Trinity Health's mission and values in every interaction.

Qualifications

• Bachelor’s degree in Nursing and three (3) years of experience with complex, high-risk inpatient populations.
• Master’s Degree in related field preferred.
• Current Iowa RN license (MercyOne Genesis, Illinois license acceptable with primary in IA or IL; second license within 12 months).
• Mandatory Reporter – Child and/or Dependent Adult Abuse training within three months of hire.
• Experience leading complex cases, care planning discussions, or interdisciplinary efforts without formal supervisory authority.

Responsibilities

• Leads interdisciplinary complex care conferences to align patients, families, and care teams around goals and discharge plan.
• Ensures patient-centered care planning by documenting and supporting patient-defined goals and informed decision-making.
• Drives accountability for next steps, responsibilities, and timelines with providers and leadership.
• Coordinates complex inpatient care through discharge, ensuring timely connection to outpatient resources.
• Engages external partners and payers to resolve barriers and support progression of care.
