Job summary

Ohio State University Physicians (OSUP) in Dublin, Ohio is seeking a Nurse Practitioner to serve as the primary clinical lead within the Skilled Nursing and Sub-Acute Rehabilitation unit. This role manages complex medical transitions for residents moving from acute hospital care to rehabilitation.

The position features a Monday–Friday, 8:00 AM – 5:00 PM schedule with on-call rotation to ensure 24/7 clinical stability for high-acuity residents. Competitive salary and benefits offered.

Qualifications

• Active Ohio APRN license and national certification (AANP or ANCC).
• Minimum 2 years in hospitalist medicine, geriatrics, or sub-acute rehab preferred.
• Proficient with EMR; strong autonomy and care-transition workflows.

Responsibilities

• Lead medical bedside care for high-acuity post-hospital residents.
• Conduct admission assessments within 24–48 hours of arrival.
• Coordinate discharge planning with interdisciplinary team.
