Job summary

UnitedHealth Group, through Optum Home & Community Care, seeks Advanced Practice Clinicians to join the Senior Community Care program in Durham, NC. You will deliver comprehensive assessments, coordinate care for high-risk members in nursing homes, senior housing, and home settings, and collaborate with an interdisciplinary team.

Preferred candidates hold ANCC/AANP or NCCPA credentials, active state licensure, and DEA authorization.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs.
• Active, unrestricted licensure in the practice state (or ability to obtain by start date).
• Active and unrestricted DEA licensure/prescriptive authority or ability to obtain post-hire.
• Driver's license and access to reliable transportation.

Responsibilities

• Conduct comprehensive assessments including medical history review, vital signs, and physical examination.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation including ICD-10 codes.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and care planning conversations.
• Provide patients and caregivers with counseling and education.
