Mass General Brigham's The Brigham and Women's Hospital is seeking a Social Worker for a primary care practice. The role emphasizes patient-centered care with biopsychosocial assessments and multidisciplinary collaboration to develop integrated care plans.

Requirements include an MSW from an accredited program and MA LICSW license, with at least 2 years in medical/psychiatric or primary care settings. The position is hybrid in Boston, with a pay range of $92,227 to $103,230 and a $10,000 sign-on

Competenze

• Master's of Social Work (MSW) from an accredited program required.
• Massachusetts Licensed Independent Clinical Social Worker (LICSW) required.
• Clinical Experience in a medical, psychiatric or primary care setting (2+ years).

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
