Beach Breeze Rehab and Care in West Palm Beach, FL is seeking an experienced MDS Director (RN) to lead the facility’s MDS program, ensure accuracy, compliance, and excellence in resident-focused care. This leadership role supports clinical documentation, reimbursement optimization, and interdisciplinary collaboration.

The position offers full-time hours, compensation of $45.00–$49.00 per hour based on experience, and benefits including medical and dental coverage, generous PTO, a 401(k) plan,

Qualifications

• Active Florida RN license.
• Minimum 2 years MDS Coordinator experience in a Skilled Nursing Facility.
• Knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning.
• Strong leadership, organizational and communication skills.
• Ability to meet deadlines in a fast-paced environment.

Responsibilities

• Oversee accuracy, completeness and timeliness of all MDS assessments and documentation.
• Ensure compliance with Medicare, state regulations, including PDPM requirements.
• Conduct resident assessments and interviews; develop care plans.
• Lead interdisciplinary care plan meetings with staff and families.
• Provide leadership and ongoing education to nursing staff.
• Schedule and manage MDS and care plans for all residents.
