Qualifications

• Active Florida RN license.
• Minimum 2 years MDS Coordinator experience in a Skilled Nursing Facility.
• Knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning.
• Strong leadership, organizational and communication skills.
• Ability to meet deadlines in a fast-paced environment.

Responsibilities

• Oversee accuracy, completeness and timeliness of all MDS assessments and documentation.
• Ensure compliance with Medicare, state regulations, including PDPM requirements.
• Conduct resident assessments and interviews; develop care plans.
• Lead interdisciplinary care plan meetings with staff and families.
• Provide leadership and ongoing education to nursing staff.
• Schedule and manage MDS and care plans for all residents.

Location: West Palm Beach, FL
Status: Full-Time
Compensation: $45.00–$49.00 per hour (based on experience)

Position Summary

Beach Breeze Rehab and Care is seeking an experienced and highly organized MDS Director (RN) to lead the facility’s MDS program and ensure accuracy, compliance, and excellence in resident-centered care. This leadership role is essential in supporting appropriate clinical documentation, reimbursement optimization, and interdisciplinary collaboration.

Key Responsibilities

• Oversee accuracy, completeness, and timeliness of all MDS assessments and supporting documentation
• Ensure compliance with state and federal regulations, including Medicare and PDPM requirements
• Conduct comprehensive resident assessments and interviews
• Develop, implement, and monitor individualized care plans for short- and long-term residents
• Lead and participate in interdisciplinary care plan meetings
• Collaborate with nursing staff, physicians, therapists, specialists, residents, and families to improve outcomes
• Provide leadership, guidance, and ongoing education to nursing staff
• Conduct in-service training on MDS, RAPS, triggers, PDPM, and Medicare guidelines
• Maintain and manage MDS and care plan scheduling for all residents

Qualifications

• License: Active, unencumbered Florida RN license
• Experience: Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning processes
• Excellent leadership, organizational, and communication skills
• Ability to manage deadlines and maintain regulatory compliance in a fast-paced environment
