• Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility.
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning processes.
• Excellent leadership, organizational, and communication skills.

Responsibilities

• Oversee accuracy, completeness, and timeliness of all MDS assessments and supporting documentation.
• Ensure compliance with state and federal regulations, including Medicare and PDPM requirements.
• Conduct comprehensive resident assessments and interviews.
• Develop, implement, and monitor individualized care plans for short- and long-term residents.
• Lead and participate in interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, specialists, and families to improve outcomes.
• Provide leadership, guidance, and ongoing education to nursing staff.

Facility: Beach Breeze Rehab and Care

Location: West Palm Beach, FL

Status: Full-Time

Compensation: $46.00–$51.00 per hour (based experience)

Position Summary

Beach Breeze Rehab and Care is seeking an experienced and highly organized MDS Director (RN) to lead the facility’s MDS program and ensure accuracy, compliance, and excellence in resident-centered care. This leadership role is essential in supporting appropriate clinical documentation, reimbursement optimization, and interdisciplinary collaboration.

Key Responsibilities
