Centene Corp. is seeking a Senior LTSS Service Care Manager RN – Pediatrics in Austin, Texas.

The role focuses on assessing and coordinating medical services for complex populations, developing personalized care plans, and educating members and families about available services. This full-time, day-shift position offers a pay range of $36–$65 per hour, with flexible work arrangements including remote, hybrid, field, or office work.

Competenze

• Graduate from an Accredited School of Nursing or a Bachelor's degree preferred.
• 4-6 years of related experience required.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required, OR NP - Nurse Practitioner - Current State's Nurse Licensure required.
• RUG certification must be obtained within 90 days of hire.

• Evaluate the service needs of complex or high-risk members and recommend a plan for the best outcome.
• Develop and continuously assess ongoing long-term care plans and collaborate with the care management team.
• Coordinate and manage communication between members, families, and care providers.
• Monitor care plans and member status, revising as necessary to meet goals.
• Review member data to identify trends and improve quality care.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect, document, and maintain member information to ensure compliance with guidelines.

About This Role

The Senior LTSS Service Care Manager RN - Pediatrics is responsible for performing care management duties to assess and coordinate medical services for complex populations, developing personalized care plans, and educating members and their families on available services.
