Job description

Immediate need for a talented Senior LTSS Service Care Manager (RN). This is a 6+ months contract opportunity with long-term potential, located in Lubbock, TX (Remote). Please review the job description below and contact me ASAP if interested.

Pay Range: $40 - $47/hour. Employee benefits include health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

  • The position is from 8 am-5 pm CST, Monday-Friday, with an hour lunch. OT is not expected during training but may be possible once a caseload is assumed and staff are managing member needs.
  • Late visits or weekend visits may be needed if requested by a family, which could result in OT.
  • Team communication occurs regularly via Teams chat, bi-weekly team meetings, and 1:1s with preceptor, team lead, and manager.
  • Daily tasks vary; you may be conducting home visits in the field or working on telephonic follow-ups from your home office.
  • Caseloads typically around 65 cases, with quarterly field visits and monthly telephonic outreaches.
  • Some cases may require daily follow-up or multiple follow-ups per day with different departments, keeping the role dynamic and engaging.

Key Requirements and Technology Experience:

  • Skills: Clinical, acute care, community, case management, bilingual in Spanish.
  • 2+ years of clinical nursing experience in a clinical, acute care, or community setting and 1+ years of case management in a managed care setting.
  • Knowledge of utilization management principles and healthcare managed care.
  • Experience with medical decision support tools (e.g., Interqual, NCCN) and government-sponsored managed care programs.
  • Experience working with people with disabilities and vulnerable populations with chronic or complex conditions in a managed care environment.
  • Education: ADN or BSN.
