Qualifications

• Must possess licensure as a Practical Nurse.
• Must have one year of experience providing practical nursing services.

• Successful completion of the employment screening process including national background investigation and drug test.

Responsibilities

• Perform medications administration and treatments per MD orders.
• Monitor and provide interventions to meet residents' needs.
• Complete accurate clinical documentation reflecting resident status.
• Supervise CNA and other care givers.
• Administer direct nursing care using nursing skills and judgment.
• Evaluate and document patient condition and needs.
• Set goals and prescribe nursing interventions for rehab and discharge planning.
• Lead team conferences and develop individualized care plans.
• Coordinate nursing care with shifts and other departments.
