Job summary

Vitality is seeking a dedicated Wellness professional to lead a compassionate care team in a busy community setting. You will administer medications, monitor residents' health, and collaborate with physicians to ensure high-quality care.

You will supervise Personal Care Specialists, support service plans, and participate in onboarding and ongoing training. The role emphasizes dignity, resident preferences, and safe, professional operations, with opportunities to influence daily care practices

Qualifications

• Must be at least 21 years old.
• Demonstrate ability to communicate effectively in English.
• Meet state health related requirements.
• Maintain current professional license if LPN/LVN.
• Maintain current CPR certification if LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Collect, store, and transport specimens for laboratory studies.
• Communicate with physicians via telephone, fax, and forms.
• Implement physician orders and ensure data is filed in charts.
• Supervise Wellness team to ensure daily duties are completed.
• Assist in developing and revising service plans.
• Report resident concerns to Wellness Director.
• Attend onboarding, annual and in-service training, and team meetings.
• Perform other duties as directed by Wellness Director.
