Job summary

Vitality is seeking a dedicated LPN/LVN to support our Wellness team in providing compassionate, safe care to residents. You will administer or assist with medications, monitor vital signs, and uphold regulatory standards while promoting dignity and independence.

You'll supervise Personal Care Specialists, help develop service plans, and coordinate with the Wellness Director. A current license, CPR certification, and English proficiency are required; at least 21 years old.

Qualifications

• Must be at least 21 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Meet state health related requirements.
• Maintain current professional license if LPN/LVN.
• Maintain current CPR certification if LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Collect, store, and transport specimens for laboratory studies.
• Adhere to medication room systems per policy.
• Communicate with physicians via telephone, fax, and Physician Communication forms.
• Implement physician orders.
• Document pertinent information in Nursing Notes and resident charts.
• Act as community supervisor when management staff is not present.
• Supervise Personal Care Specialists to ensure daily duties are completed.
• Assist in development and revision of service plans.
• Inform Wellness Director of current resident needs and preferences.
• Read Communication Log and Shift Change Notes, updating as required.
• Attend onboarding, annual and in-service training and team meetings.
