Responsibilities

• The LPN participates in planning, implementation and evaluation of nursing care using focused assessment.
• Communicates with the care team when patient needs exceed the LPN scope or knowledge.
• Provides teaching to patients and families and supports orientation of new staff.
• Collaborates with the health care team to improve patient outcomes and maintains patient safety.
