Job summary

Vitality Living in Franklin, TN is seeking a Director of Wellness to lead a compassionate team delivering personalized care for residents. The role ensures safe nursing operations, regulatory compliance, and strong family communication.

The position requires an active RN/LPN license, CPR certification, 2–5 years in skilled nursing or home health, and demonstrated leadership. Benefits include comprehensive health coverage, retirement plan, and PTO.

Qualifications

• Current RN or LPN/LVN license in good standing.
• CPR certification and all required state health qualifications.
• Bachelor’s degree in health, human services, or related field.
• 2–5 years of experience in a similar role in skilled nursing, home health, and/or rehabilitation serving older adults.
• Proven team leadership experience.

Responsibilities

• Recruit, hire, supervise, and develop the Wellness team, ensuring ongoing training and performance management.
• Assess prospective and current residents and oversee individualized care plans addressing physical, emotional, social, and spiritual needs.
• Oversee medication management, physician orders, laboratory results, and treatments with timely follow-up.
• Monitor changes in resident health, review incidents, and coordinate communication with families, physicians, and healthcare partners.
• Maintain quality care systems, conduct audits, and report wellness indicators for continuous improvement.
• Ensure compliance with regulations and scope-of-practice while protecting resident dignity and confidentiality.
• Provide clinical guidance and nursing coverage as needed, coordinating services with home health, hospice, and other providers.
• Participate in community leadership, tours, and emergency preparedness to maintain a safe, professional environment.
