Otterbein SeniorLife seeks a Social Worker to enhance residents' quality of life with strong psychosocial support for residents, families, and partners. You will contribute to care planning, supervise grievance processes, and ensure timely MDS and discharge documentation, while supporting transitions and education on abuse and neglect.

Qualifications

• Education: post-secondary preferred.
• Licensure: Michigan Licensed Social Worker.
• Experience: 2 years in a nursing home setting preferred.

Responsabilités

• Know each resident and their family with psychosocial profile.
• Participate in care planning for resident needs.
• Ensure activities are available to residents outside facility.
• Complete MDSs timely and accurately.
• Lead the Grievance Committee and address concerns.
• Coordinate investigations with the Executive Director.
• Provide annual abuse, neglect, misappropriation education.
• Support transitions of care and Medicaid waiver processes.
• Case management and admission/discharge documentation.
