Job summary

Casa de las Campanas in Rancho Bernardo, San Diego, seeks a Residential Social Worker to coordinate social services for residents across Independent, Assisted Living and Memory Care units. You will develop and implement a case management program, provide education to staff, and establish a comprehensive support plan for residents and families.

Qualifications

• Bachelor’s degree in Social Work/Psychology/Gerontology or related field.
• Minimum two years’ experience with geriatric residents in Long Term Care.
• Knowledge of Title 22 Regulations and case management.
• Ability to administer Depression Scale and Cognitive Testing.

Responsibilities

• Develops and implements a case management program for psychosocial support.
• Assist residents in transitions between care levels and support families.
• Provide resources on grief, loss, caregiving, hospice, and aging concerns.
• Input for departmental budgets and revenue from counseling programs.
• Coordinate educational sessions for residents on age-related topics.
• Collaborate with leadership to develop a holistic program for residents.
• Assess residents’ cognitive, functional, and psychosocial abilities annually.
• Serve as liaison for residents in skilled nursing.
