Job summary

Seminole Care & Rehabilitation Center is seeking a qualified Social Services Supervisor to coordinate discharge planning and psychosocial care for residents. You will develop measurable plans, assess residents and families, and ensure safety across care teams.

The role emphasizes collaboration, confidentiality, and staff training to uphold ethical practices and residents' rights while maintaining strong ties with medical colleagues.

Qualifications

• Bachelor’s degree in social work or related field.
• Correlation with health care social work experience preferred.
• Current State Clinical Social Work Licensure.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families at all organizational levels.
• Ability to react decisively in emergency situations.
• Ability to maintain confidentiality.
• Ability to read, write and speak English.
• Knowledge of training techniques for clinical staff.
• Good ethical practice.
• Relationship management.

Responsibilities

• Coordinate social service/discharge planning within the hospital system through effective communication at all levels.
• Develop measurable plans and objectives to enhance social service daily.
• Perform psychosocial assessment and reassessment for residents and address family needs.
• Ensure resident safety and reduce health care errors in care delivery.
• Involve family in teaching and care planning to support resident goals.
• Interpret and promote residents’ rights and the Residents' Bill of Rights.
• Provide individual and group support to residents and families during adjustment or crisis.
• Direct the Social Service/Discharge Planning Department to meet resident needs and follow center policy.
• Provide resources and support to Social Service/Discharge Planning staff and clinical staff.
• Maintain rapport with medical staff and other departments.
