Responsibilities

• Lead and oversee Social Services operations, ensuring high-quality, resident-centered care and regulatory compliance.
• Direct and participate in resident assessments, care planning, and discharge planning activities.
• Develop and implement individualized social service plans that support resident goals and quality of life.
• Collaborate with residents, families, and community partners to develop programs that enhance engagement and well-being.
