Job summary

Sprenger Health Care in Lorain, OH seeks a dedicated social worker with at least 2 years’ experience in long-term care. A licensed social worker or a bachelor’s degree is preferred, with excellent communication and organizational skills essential to support residents and families.

Essential duties include building one-to-one relationships with residents and families, coordinating admissions and discharges, liaising with hospice and community agencies, and supporting referrals for ancillary

Qualifications

• Two years of experience in long-term care.
• Licensed Social Worker or bachelor’s degree preferred.
• Excellent communication and organizational abilities required.

Responsibilities

• Develop one-to-one relationships with residents and families.
• Establish relationships with area hospice and community agencies.
• Act as liaison with social, health and community services.
• Coordinate with Activity Department to meet social needs.
• Oversee referrals for ancillary services (dentist, eye doctor, podiatry, psychology).
• Assist in admissions and discharge processes as needed.
