Prava Therapy is seeking a full-time Speech-Language Pathologist to join our premier memory care and assisted living community in Frederick, Maryland. You will deliver comprehensive evaluations and evidence-based plans of care for adult and geriatric clients, collaborating with an interdisciplinary team.

This role emphasizes functional outcomes, avoidance of complications, and real-world communication and swallowing support.

Responsibilities

• Perform comprehensive evaluations and develop individualized, evidence-based care plans for adult/geriatric clients.
• Assess and treat cognitive-communication, aphasia, dysarthria, dysphagia, AAC, and related disorders.
• Collaborate with physicians, dietary and wellness teams to support holistic care.
• Monitor progress and modify plans; maintain compliant, timely documentation.
• Contribute to discharge planning and interdisciplinary team conferences.
