Job summary

Christian Living Communities (CLC) and Cappella Living Solutions (CLS) seek a dedicated professional to inform residents and families about health and support services while developing partnerships with service providers for resident referrals.

The role includes educating residents on available services, monitoring provisions, and fostering a continuum of care to support autonomous and engaged aging in place.

Qualifications

• Experience in social work, healthcare, or gerontology with older adults.
• Minimum 2 years direct service experience with older adults, in assisted or independent living preferred.
• Able to respond effectively in crisis situations.
• Excellent customer service and communication skills.

Responsibilities

• Inform residents and families about available health and support services.
• Develop contacts with service providers and agencies for referrals.
• Educate residents on services and monitor provision of services.
• Foster a continuum of care and coordinate necessary assistance.
• Maintain resident files and respect resident rights.
