Responsibilities

• Respond to residents in need of psychosocial or supportive services and refer as needed.
• Conduct assessments of potential residents to determine suitability.
• Recommend appropriate residency levels and coordinate care planning.
• Assess and resolve issues regarding residents’ care and safety.
• Develop referral networks within the community for available services.
• Educate residents on Advanced Directives and document completion.
