Job summary

Integral Senior Living is seeking a Resident Care Coordinator to support the Director of Health & Wellness in delivering outstanding care within Assisted Living and Generations/Memory Care units. You will coordinate care services, supervise staff, monitor compliance with state/federal policies, and contribute to staff development while ensuring a safe, compassionate environment for residents and families.

The role emphasizes collaboration with caregivers, medication aides, and medical providers

Qualifications

• High school diploma or equivalent required.
• Certifications as required by the state.
• Prefer 1 year experience providing residential care to the elderly or dementia care.
• Prefer 6 months supervisory experience.
• Prefer LVN license.

Responsibilities

• Ensures provision of services to residents and that all Federal and State policies governing resident care are followed.
• Interview, hire, evaluate, coordinate, motivate, monitor staff in accordance with policy.
• Coordinate departmental schedules and ensure adequate staffing per standards and state law.
• Oversee continuing education and staff development programs for caregivers and medication aides.
• Conduct orientation for new department members to ensure understanding of goals and procedures.
• Coordinate medication provision and ensure proper documentation; supervise central storage and delivery.
• Assist with resident assessments and pre-admission assessments and monitor changes in status.
• Develop methods to coordinate nursing services with other resident services for continuity of care.
• Act as care manager and liaison for residents and families; obtain orders and coordinate with physicians.
• Coordinate prescription orders with doctors and pharmacy delivery; assist with medication-related documentation.

Education
