Job summary

Heritage Crossings is seeking a Physical Therapist Assistant (PTA) to provide high-quality rehabilitation on a PRN basis in a supportive senior living environment. You will work with an experienced therapy team to help residents improve mobility and independence.

Requirements include graduation from an accredited PTA program and an active Nebraska PTA license, plus strong clinical and communication skills for collaboration with nursing staff and families.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active Nebraska Physical Therapist Assistant license.
• Strong clinical, communication, and organizational skills.
• Ability to build positive relationships with residents, families, and team members.
• Desire to provide person-centered care with professionalism and compassion.

Responsibilities

• Implement individualized treatment programs developed by the supervising Physical Therapist.
• Support residents in reaching their rehabilitation goals through therapeutic exercises and functional mobility training.
• Monitor resident performance and communicate progress, concerns, and recommendations to the therapy team.
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
• Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.
• Encourage resident participation while fostering confidence, safety, and independence.
