Job summary

Heritage Crossings Geneva is seeking a dedicated Physical Therapist Assistant to provide high-quality rehabilitation services on a PRN basis. You will help residents improve mobility, restore function, and achieve meaningful outcomes in a supportive senior living environment.

Join a collaborative therapy team to implement treatment plans, monitor progress, document outcomes, and communicate with the team to ensure safe, person-centered care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active Nebraska Physical Therapist Assistant license.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Implement individualized treatment programs developed by the supervising Physical Therapist.
• Support residents in reaching their rehabilitation goals through therapeutic exercises and functional mobility training.
• Monitor resident performance and communicate progress, concerns, and recommendations to the therapy team.
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
• Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.
• Encourage resident participation while fostering confidence, safety, and independence.
