• Current Physical Therapy licensure in state of practice.

• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient to determine rehab potential and sets goals in the team plan.
• Sets initial STG's and LTG's and develops an individualized plan of care.
• Supervises PTA/tech and documents services per department standards.
• Develops a home exercise program and provides patient/family instruction.
• Participates in team and family conferences and PI programs.
