Job summary

UnitedHealth Group, through LHC Group, seeks a Senior Living Physical Therapist to provide age-appropriate services for adults to geriatrics, including assessments and direct treatment in outpatient settings.

You will supervise PTA/tech, develop home exercise programs, instruct patients and families, participate in team conferences, select adaptive equipment, and document services per department standards.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient to determine rehab potential and develops an individualized plan of care.
• Sets initial short- and long-term goals.
• Maintains and logs equipment condition and safety checks.
• Follows prescribed medical regimens and safety precautions.
• Performs safe treatment techniques including transfers and therapeutic modalities.
