Job summary

UnitedHealth Group is hiring a Senior Living Physical Therapist to provide age-appropriate care for adult to geriatric patients in an outpatient setting. You will conduct assessments, deliver treatment, supervise PTA/tech, and develop home exercise programs in alignment with department standards.

Responsibilities include establishing treatment goals, selecting safe techniques, documenting progress and participating in PI activities and team conferences.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• Preferred CPR certification.

Responsibilities

• Evaluate the patient to determine rehab potential and develop an individualized plan of care with short- and long-term goals.
• Set initial STG's and LTG's for patient progress.
• Utilize treatments and equipment safely; maintain logs and report equipment issues.
• Complete discharge summaries and ensure proper documentation for outpatient PT.
• Collaborate with PTA/tech and participate in team and family conferences; provide home exercise programs.
