Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for home visits with our Ocean County Senior Living team. This role involves working with clients in senior living communities throughout Ocean County on a per-visit basis.

As a home care Physical Therapist, you will be an essential part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home with appropriate medical support.

BAYADA Senior Living offers comprehensive care, including traditional home care under Medicare Part A and outpatient therapy services under Medicare Part B. Our focus is on exceptional clinical care and relationship management, partnering with senior living communities such as assisted living, independent living, congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities.

Responsibilities:

1. Make home visits to clients within designated geographic areas.
2. Perform diagnostic tests and measurements related to mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans, including manual exercises, gait training, balance, and other therapies.
5. Assess and update care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a touchpad tablet.

Qualifications:

1. Current NJ Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program or equivalent, evidenced by transcript or diploma.
3. Ability to work independently and manage time efficiently.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records (EMR) preferred.
