Summary

We are hiring for a Senior Living Lead Therapist to join our team in Senior Living.

Pay Range: $47 - $51/hour + opportunity for additional pay codes.

The Senior Living Lead Therapist is responsible for:

  • Day-to-day clinical care of patients on HH and Outpatient Services.
  • Processing orders and attending clinical meetings within the facility.
  • Operationalizing all components of the SL plan in their facility/facilities.
  • Reporting to SLD for all operational, growth, quality, and compliance matters in their facility/facilities.

Responsibilities

  • Document all Outpatient and HH visit types per company standards.
  • Ensure timely initiation of care for Outpatient and HH SOC.
  • Schedule patients at appropriate times for therapy.
  • Identify patient and family education needs.
  • Collaborate with the team to establish interdisciplinary team goals.
  • Utilize equipment properly and safely; report broken or faulty equipment.
  • Follow medical regimens developed for the patient, adhering to precautions (THR, TKR, Cardiac, Wound, etc.).
  • Use safe treatment techniques: transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, etc.
  • Apply treatment approaches according to established PT protocols and policies.
  • Implement alternative or innovative approaches as needed.
  • Process and secure physician orders.
  • Be aware of the financial situation, plan care accordingly, and ensure patient awareness of needed equipment and home accessibility. Design home exercise programs and make appropriate referrals at discharge.
  • Serve as a member of the Performance Improvement team.
  • Participate actively in Department Team meetings.
  • Conduct in-services for staff as needed.
  • Participate in weekend and holiday rotations as required.
  • Supervise students as indicated.
  • Assist in interviewing new therapy department employees.
  • Ensure treatments and documentation meet all company standards.
  • Attend weekly Team Conferences and participate actively in the ID team.
  • Proficient in wound care as allowed by state practice acts.

Education and Experience

  • Must be a currently licensed Physical Therapist, Physical Therapy Assistant, Occupational Therapist, Occupational Therapy Assistant, Speech Therapist, or Licensed Practical Nurse in at least one responsible state.
  • Recent graduates applying for the registration exam may be considered for provisional appointment.
  • CPR certification required.
  • Current Physical Therapy License in the state of practice required.
