Legacy Healthcare Services, Inc. seeks a Physical Therapist to join our team in North Carolina.

You will provide rehabilitative services to senior residents under medical orders within a coordinated care team, and develop initial and ongoing treatment plans tailored to each patient’s clinical findings. You will participate in conferences to coordinate care, and deliver one-on-one sessions to ensure individualized attention and functional based treatment.

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsabilités

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one with private treatment sessions and individualized attention.
• Adapt treatment sessions to individual resident needs and safety requirements.
