Job summary

LHC Group, a leading post-acute care partner within UnitedHealth Group, is seeking a Senior Living Physical Therapist for a full-time role in Massachusetts. You will deliver age-appropriate PT services to adult and geriatric patients, perform assessments, develop home exercise programs, and supervise PTA/tech staff, documenting care per facility standards.

Requires current physical therapy licensure in the state, a valid driver's license, and reliable transportation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient to determine rehab potential and develops an individualized treatment plan.
• Sets initial short- and long-term goals.
• Safely uses equipment and maintains logs for equipment inspection and cleaning.
• Delivers direct patient care and supervises PTA/tech as needed.
• Develops home exercise programs and provides patient/family instruction.
• Participates in team/family conferences and documentation per standards.
