Job summary

LHC Group is seeking a Senior Living Physical Therapist to provide age-appropriate services for adult to geriatric patients, including assessment, direct treatment, patient/family instruction, and documentation per department standards. This role involves supervision of support staff and participation in team conferences.

Required qualifications include active Physical Therapy licensure in the state and a valid driver’s license with access to reliable transportation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient to determine rehab potential; creates an individualized treatment plan and goals.
• Sets initial short- and long-term goals.
• Uses safe treatment techniques (transfers, positioning, splinting, ultrasound, etc.).
• Supervises PTA or tech delivering treatment and documents services.
• Develops home exercise programs and provides patient/family instruction.
• Participates in team and family conferences and PI programs.
• Completes discharge summaries on final visits in outpatient PT.
